Staff Nurse (Band 5) – Waverley Reablement Unit

Bridgwater Community Hospital is looking for a passionate and proactive Staff Nurse to join our joint-led team of Therapists and Nurses. In this role, you will be at the forefront of the HomeFirst Pathway, a modern model of care designed to bridge the gap between hospital and home. At the Waverley Reablement Unit, we don’t just treat patients; we empower them to regain their independence in a state-of-the-art facility, ensuring their return home is safe, swift, and sustainable.


Core Requirements and Skills

  • Professional Registration: Must be a Registered Nurse (Adult) with a valid NMC PIN.

  • Clinical Assessment: Ability to assess complex patient needs and develop, implement, and evaluate personalized treatment plans.

  • Multidisciplinary Working: Strong collaboration skills to work alongside therapists, social care colleagues, and hospital teams.

  • Rehabilitation Focus: A pro-active attitude toward supporting patients recovering from surgery or acute illness.

  • Communication: Exceptional skills in engaging with patients and carers to co-produce care goals.

  • Independence Advocacy: A commitment to promoting self-care and confidence in patients prior to discharge.


Key Responsibilities

  • Holistic Care Delivery: Manage a caseload of patients on the Reablement Unit, focusing on both clinical stability and functional recovery.

  • HomeFirst Integration: Actively participate in the HomeFirst model, coordinating with Musgrove Park Hospital to minimize hospital stay duration.

  • Patient Education: Teach and support patients to manage their own health needs, ensuring they are prepared for a “safe and proactive” discharge.

  • ** Multidisciplinary Coordination:** Lead and participate in ward rounds and MDT huddles to ensure care is tailored and efficient.

  • Skill Development: Engage in ongoing training to adapt to a changing, community-focused NHS landscape.


The HomeFirst Pathway & Reablement Model

The HomeFirst approach is a “discharge to assess” model. It operates on the principle that the best place for a patient to finish their recovery and undergo assessment for long-term needs is in their own home, rather than an acute hospital bed.

By working on the Waverley Reablement Unit, you are the critical link in this chain. You provide the short-term, intensive clinical support and therapy required to get a patient stable enough to transition to the next phase of their recovery at home.
