Healthcare Assistant (Community Overnight Service)

About the Role
As a Community Healthcare Assistant, you will be the reassuring presence for patients and their families during the overnight hours. Working across various home settings, you will provide essential personal care, emotional support, and safety monitoring. Because you are working in a community capacity, you will have the autonomy to manage your overnight visits while remaining connected to a wider clinical support network. Your role is critical in preventing hospital admissions and ensuring a high quality of life for those with complex or end-of-life care needs.
Key Responsibilities

Compassionate Care: Providing high-quality personal care, including assistance with positioning, hygiene, and comfort management.

Safety & Monitoring: Maintaining a vigilant presence throughout the night to ensure patient safety and responding promptly to any changes in condition.

Documentation: Accurately recording care activities, observations, and incidents in digital or paper-based patient logs to ensure continuity of care.

Preserving Dignity: Carrying out all duties with a deep respect for the patient’s privacy, choices, and personal environment.

Communication: Providing clear handovers to daytime staff and family members, and escalating concerns to the on-call clinical lead when necessary.
Requirements and Skills

Experience: At least 6 months of experience in a health or social care setting (e.g., care home, hospital, or home care).

Nurturing Approach: A naturally empathetic demeanor with a passion for supporting vulnerable individuals.

Licensing: A full UK driving license is essential for traveling between patient homes in the community.

Reliability: The ability to work consistently through night shifts and maintain high standards of care during late-night hours.

Communication: Strong verbal and written skills for accurate reporting and interacting with patients and their families.
