Key Responsibilities

The Triage Administrator serves as the “filter” for the community nursing service, ensuring that every patient is directed to the right care at the right time.

  • Referral Management: Action and process referrals on electronic clinical systems, ensuring patient demographics are accurate and outcomes are planned within scope.

  • Clinical Liaison: Identify urgent calls that require nursing expertise and forward them immediately to the Clinical Triage Nursing Team.

  • Data Integrity: Ensure a minimum standard of information is recorded for all messages; this involves proactive follow-up with referrers or patients to fill missing data gaps.

  • Service Improvement: Collaborate with the PALS (Patient Advice and Liaison Service) to capture user feedback and assist in driving service enhancements.

  • Team Support: Participate in daily huddles and team meetings (including minute-taking) and support the induction of new starters by maintaining the team handbook.
