Job Overview:
As a HART Support Worker, you will deliver short-term, intensive reablement support to people in their homes, helping them recover from illness, injury, or disability. You will assist with daily living tasks, promote independence, and support community engagement, working across Leicestershire on a flexible shift rota.

Requirements:

  • Level 2/3 Diploma in Health & Social Care or equivalent experience

  • Committed to delivering high standards of care in an anti-discriminatory manner

  • Effective team player and able to work independently

  • Basic IT skills to maintain records

  • Excellent communication and observation skills

  • Ability to remain calm under pressure and work flexibly

Responsibilities:

  • Provide personal care including washing, dressing, showering, and bathing

  • Assist with meal preparation, domestic tasks, and community engagement

  • Motivate clients to achieve their independence goals

  • Support with assistive technology and safe use strategies

  • Prevent falls and provide guidance on managing fatigue and pain

  • Maintain accurate records and observations
