Birmingham Community Healthcare NHS Foundation Trust (BCHC) is seeking a highly motivated and patient-centered Community Nurse to join our established planned care team based in Stechford. This role is dedicated to delivering high-quality, integrated clinical care to patients within their own homes across four council wards surrounding Heartlands Hospital. As a community-based practitioner, you will be the vital link between patients and the wider multidisciplinary team, ensuring independence, dignity, and wellbeing for those you serve.
Key Requirements

Mobility: A full UK driving license and access to a car for work purposes are essential for this community-based role.

Clinical Competence: Proficiency in a wide range of clinical skills, including diabetes management/insulin administration and complex wound care.

Compassionate Care: Experience or a strong interest in providing palliative and end-of-life care for patients wishing to remain at home.

Flexibility: Ability to work within a service that operates 7 days a week (8 am to 6 pm), including weekends and bank holidays.

In this role, you will act as a primary clinician and patient advocate, navigating the challenges of the home setting to deliver hospital-standard care. Your day-to-day responsibilities involve managing a diverse caseload—from administering life-saving medications and performing complex dressing changes to coordinating with GPs, social workers, and therapists. You will be a specialist in “person-centered care,” ensuring that every intervention is tailored to the patient’s environment and preferences. Whether you are managing a chronic condition or supporting a family through the final stages of a loved one’s life, your clinical expertise and empathetic communication will be the cornerstone of the community healthcare experience.
